Iowa requires insurance to cover autism treatment

Iowa Code § 514C.28 (state employee plans) and § 514C.31 (group policies)

Ages:
Section 514C.28 sets no age limit. The separate ABA requirement at 514C.31, which reaches group policies generally, applies to covered individuals under 19.
Limits:
No dollar cap in the current text of 514C.28, and coverage is expressly not subject to any limit on the number of visits to an autism service provider.

Self-funded employer plans (ERISA) are typically exempt — ask your HR department which type of plan you have.

How autism qualifies

Iowa runs no autism-specific waiver. Your county case manager picks from the general waivers — Intellectual Disability, Children's Mental Health, or Health and Disability — based on your child's clinical picture. That makes the case manager unusually powerful here: which waiver they route you to determines both what you get and how long you wait, so it is a conversation to have deliberately rather than passively.

How the waiting list is ordered

The Intellectual Disability waiver is the largest, with 12,427 participants, and waits run into years — some applicants have been waiting since 2020. One feature of Iowa's process deserves a warning, because it is unusual and it costs families badly: Iowa does not pre-screen for eligibility before placing applicants on a waiting list. You can wait years and only then discover you never qualified. Do not treat being on the list as evidence that you are eligible. Ask your case manager directly whether your documentation would meet the level-of-care standard today, and fix any gaps now rather than in five years.

Contact your county case manager, who is the single point of entry to the whole system. Ask which waiver you are being considered for and why, and ask them to confirm your level-of-care documentation is adequate before you settle in to wait.

Federal Medicaid rules (EPSDT) require medically necessary treatment, including ABA, for enrollees under 21 in every state. We did not find Iowa publishing its own ABA policy, so ask your plan what it covers beyond that.

Most Iowa Medicaid members are in Iowa Health Link and get coverage through one of three managed care organizations, and Iowa HHS rulemaking states that ABA services are covered as a Medicaid benefit. You have 90 days from your enrollment date to switch plans for any reason, and after that once every 12 months during your Annual Choice Period. Some members are in fee-for-service instead of managed care.
